This guide is to report a user who has either separated from Genentech / Roche or have changed roles in the company. It is the responsibility of managers or a designated delegate to inform the ACE Infra team of a separation or role changes.

These are critical steps in maintaining our security posture and protecting patient data and can adversely impact our security posture and result in compromise of patient data if not done in a timely manner.

Procedures

1. Go to the Github ACE Infra Issues project and create a new issue using this link to select the User Offboarding Template.
2. Areas that requires your input are denoted by < > or ( ). For example, the title has a place holder for you to enter the unix id of the person who is separating from ACE. Update these placeholders, then click on “Submit new issue”. No need to select labels. The labels will be added automatically based on the template.
3. Please complete the section designated to be completed by the person open the ticketing. Do not complete the section below that titled The information below is to be completed by the ACE Infra team.
4. Effective data is the data the separation or role change goes into effect.
